What are advance care plans?
Advance care planning sets out your choices, giving your whanau and healthcare team a plan to follow if you are later unable to make your own decisions understood.
We can guide you in the kinds of conversations you need to have, provide an advance health care plan form for you to complete, and give you more information about your options for future health care.
The benefits of advance care plans
- A record of what you want, so it’s clear what health care you want, even if you later become unable to express yourself
- A clear plan that you can show to your whānau and health care team
- Peace of mind for the future – whatever happens, your wishes are spelled out
- Advance care planning in end of life care can also make your final days less upsetting for your loved ones
- Everything will be captured on an NZ Advance care plan form
Some questions to consider
Do you want to know only the basics about your health conditions, or all the details?
If your doctor could make an estimate for how long you had to live, would you want to know?
How much information do you want your loved ones to receive about your condition?
Would you like to appoint a loved one to make decisions on your behalf (an Enduring Power of Attorney)?
If you are dying, where would you like to be?
Would you like to be noted down as an organ donor?

如您想在我们的诊所注册，您先需要填写我们初级卫生保健机构（Primary Health Organisation，简称 PHO）的注册表格。这是新西兰卫生部的规定。您还需要提供身份和资格证明：
- 新西兰护照或新西兰出生证明，或
- 其它国家的护照与永久居民签证、居民签证、或 24 个月以上的工作签证
在注册表格上请注明你想注册在哪位还有接受新病人的医生名下。
在我们诊所新注册的病人需要约见我们的护士或医疗助理完成一个健康询问会诊。
在健康询问会诊中，护士将会测量您的身高、体重、血压等，和简单的跟您相谈病史，让我们的医疗团队更好了解您的健康状况。
完成健康询问会诊后便可以与医生会诊。
All GP appointments are 15 minutes long.
- One appointment is for a single health issue.
- Should you wish to have a 30-minute GP appointment, it will be charge at a non-subsidized rate.
- Depending on your age and health, one of our specialist nursing or pharmacist clinicians may be able to spend more time with you.
If you are a new patient, you will get full access to the patient portal “My Indici” once you have completed your enrolment.
