如您想在我们的诊所注册，您先需要填写我们初级卫生保健机构（Primary Health Organisation，简称 PHO）的注册表格。这是新西兰卫生部的规定。您还需要提供身份和资格证明：

• 新西兰护照或新西兰出生证明，或
• 其它国家的护照与永久居民签证、居民签证、或 24 个月以上的工作签证

在注册表格上请注明你想注册在哪位还有接受新病人的医生名下。

在我们诊所新注册的病人需要约见我们的护士或医疗助理完成一个健康询问会诊。

在健康询问会诊中，护士将会测量您的身高、体重、血压等，和简单的跟您相谈病史，让我们的医疗团队更好了解您的健康状况。

完成健康询问会诊后便可以与医生会诊。

All GP appointments are 15 minutes long.

• One appointment is for a single health issue.
• Should you wish to have a 30-minute GP appointment, it will be charge at a non-subsidized rate.
• Depending on your age and health, one of our specialist nursing or pharmacist clinicians may be able to spend more time with you.

If you are a new patient, you will get full access to the patient portal “My Indici” once you have completed your enrolment.

请在会诊后尽快支付诊费。没有付清的款项每月将会有 $10 额外行政费用。

任何其它额外治疗、化验、材料等，另行收费。

我们建议您通过您的 MyIndici 患者平台预约。.

如果您有紧急状况需要会诊，请致电诊所。
